
You will be asked to provide consent prior to any and all assessments and treatments provided by Elev8 Physiotherapy. You will be asked to complete a consent form during your initial assessment during which we will establish your medical history, medications and current baseline activity levels.
You may be asked to remove part of clothing in order to assess and treat you. If you do not feel comfortable doing this, please inform your Physiotherapist.
Please note that not all treatments may be appropriate for every patient but your Physiotherapist will establish with you the recommended treatment plan following your initial assessment. If treatment cannot be offered through our Physiotherapy service, you may be referred on to another healthcare professional with your consent.
You may be advised by your Physiotherapist to purchase inexpensive equipment or aids to facilitate your recovery. This is not compulsory and can be discussed.
All fees will be discussed and agreed with you prior to assessment and treatment. You will receive an invoice following your appointment which can be paid via cash or BACS transfer.
The fees must be paid within 7 days from the date the invoice was sent, and failure to do so will result in termination of services.
We understand that in some situations appointments may need to be cancelled or rescheduled due to unforeseen circumstances. If you cancel within 24 hours of your appointment or you are not present at the agreed appointment location, you will be liable to pay for the session.
You are eligible to stop a session at any time during the appointment, likewise your Physiotherapist may terminate a session if they feel it is appropriate to do so. Please note you may still be liable to pay for the cost of that appointment.
Your personal and medical information is private and all information provided will remain confidential. This means we will not share your information with third parties, unless you provide consent to do so. Please note however where required by law, we may need to provide information to additional parties to protect yourself or others.
